To Our Patients

The care and comfort of our patients is of utmost importance to us. As such, we want to make sure that our new patients are prepared for their first appointment.

Below is a brief overview of what to expect.

New Patient Procedures

A new patient appointment will last an hour and a half. During this time, the patient (and their guardian, if applicable) will meet with Dr. Dietz and have a complete dental exam and cleaning. A full set of x-rays will be taken, including a Panorex and four bite wings. Please bring all current x-rays with you. Dr. Dietz prefers a Panorex x-ray no more than five years old. After you have scheduled your appointment with our office, please print, review and fill out the forms below and bring them with you to your appointment. If you are unable to download the forms, please let us know and we would be happy to mail you a copy.

Policies

Our staff has made a promise, professionally and personally, to devote the time, respect and care to meet your individual needs and make your visit a comfortable and pleasant one. Please review our policies below to prepare for your next appointment. We look forward to continuing to meet your dental needs in the future.

  • If you should need to cancel your appointment, we ask that you notify us at least 24 hours in advance. Failure to do so will result in a $75.00 missed appointment fee. Of course we do understand that flat tires, sick children and family emergencies do happen and a true emergency which causes you to miss an appointment will be handled on an individual basis.

    **Please note the above policy also applies to new patient appointments.

  • We collect payment at the time of service for patients both with and without dental insurance. This policy is firm and without exceptions. We accept the following forms of payment: cash, checks, MasterCard, Visa and Care Credit.

  • We file insurance for patients with dental coverage and your insurance company will reimburse you directly.
